You can get the Bloodletter as a reward for completing the Blood Tide quest. Talk to Cassie Dalton in Far Harbor and she’ll ask for your help. You’ll have to visit several different places and kill a handful of specific enemies for her to get her revenge. Once it’s all done, she’ll give you the weapon, a family heirloom which she claims the island will recognize.
