This place is southwest of the Earthborne Cave site of grace and northeast of Tombsward Catacombs one. Basically in the middle of them. You can see the exact location in the screenshots below. The exact place you’re looking for is around the middle of the ruins, where you’ll find what seem to be the remains of a church. In the church, you’ll find a giant hag skulking in the northern end, and a whole bunch of her underlings around her. She’s the queen, and you have to kill her to get the item in question.
